The Disqualification Registry contains the names of individuals who are disqualified from working with consumers receiving services from the Department of Mental Health. The lists consists of individuals with a substantiated charge of:

Individuals that desire in-home services for a child, senior or person with disability may request a screening through the Department of Mental Health's Disqualification Registry by completing the Caregiver Background Screening form. On average, information from the registry takes 2-3 working days to process from the date of receipt of the request.
