2007 Annual Report to CMS
From January - March 2006, Transformation Initiative staff worked with the Transformation Steering Committee, Transformation Workgroup Participants and DMH staff to draft the Transformation Initiative Strategic Plan. A draft of the strategic plan was submitted to the Center for Medicare and Medicaid (CMS) on May 2nd, 2006 and was discussed with CMS on May 15th. On June 23, 2006 a final draft, based on feedback from CMS, was submitted through a web-based application and therefore varied slightly from the original template previously posted on this web page. The strategic planning documents listed below contain all information submitted through the web based application, although a few of the exhibits have been merged for the convenience of the reader.
Face Sheet : Includes the strategic plan review process and a list of stakeholders. The list of stakeholders corresponds to our list of steering committee and workgroup members.
Structure and Process: Includes a graphic depicting how key participants in the process relate to each other as well as a brief description of the graphic.
Mission and Vision Statements: The Mission Statements and Guiding Values have been developed through stakeholder discussion.
System Readiness Update: An updated / revised synopsis of the System Readiness Assessment that was submitted with our original application.
Strategic Plan Detail: This document outlines the following:

- Outcomes / Outcome Measures and
- Role of Consumers and Families
Implementation Charts: Shows a graphic depicting the relationship between the responsible entity or entities for each goal. This is a graphic representation of the way in which implementation will be organized. Links to the graphs follow: Goal 3 (QA) - Goal 4 (IT) - Goal 5 (CS) 
Implementation Timeline: An approximate timeline of year one activities.
Technical Assistance Plan: The Technical Assistance Plan identifies areas where technical assistance may be needed.
